Abstract

Continuity of Care (CoC) is an approach that emphasizes the sustainability of midwifery services from early pregnancy to postpartum, by involving close communication between midwives and pregnant women, and supporting family involvement in the process. The goal of CoC management is to improve the quality of service, reduce the number of complications, and increase patient satisfaction by ensuring that mothers receive consistent and personalized attention. The research design used is descriptive and case study research type. Guidelines for observation, interviews and documentation studies in the form of midwifery care formats starting from pregnancy, childbirth, postpartum and newborn. Through this approach, midwives can provide integrated care, monitor the health development of the mother and fetus continuously, and provide appropriate education according to the patient's condition and needs.
